Over the years, it has become a popular option for removing facial hair as well. But in rare cases, lasers can actually increase thick hair growth, sometimes many months after beginning treatments and usually in an untreated area adjacent to the location being treated. The distressing complication, called "paradoxical hypertrichosis" is most likely to occur in Mediterranean women along the jawline, says Dr. Bernstein, "although it can happen in men and women in any location." Hormonal issues, such as polyscystic ovarian syndrome, are often the culprit. "If the hair is dark and coarse and there are no hormonal issues, it responds well to laser treatments," Dryden says. "But if it is fine or light, we have to get more aggressive with the settings, and it will typically take more treatments." "Sometimes on the outer corner of the upper lip and on the chin, there are a couple of hairs that are more stubborn," says Dr. Martin. "For these, you just switch back to the single pulse mode and go with higher energy just to effect those isolated hairs." When working on the face, Dr. Bonakdar pre-treats the patient with anesthetic cream and uses ice for immediate cooling. Setting Client Expectations Though laser hair removal is one of the most popular procedures in medspas and cosmetic practices, the range of outcomes can lead to dissatisfaction. Patient education and realistic expectations are key to positive outcomes. "I spend time educating my patients that between three to five treatments are needed before permanency is reached," Dr. Bonakdar says. "If the patient is hairier than the average person, I prepare them to expect to do more than five treatments. I explain that 'permanent' hair removal is defined as achieving six months hair-free. This means that future touch-up treatments are needed from time to time." Dr. Bernstein agrees: An educated patient is a happy patient. "The best way to improve patient satisfaction is to set lower expectations than you really have, because patients often only hear the positives."
